With 5.6 I've seen it with the traditional secondary databases.
With RC1 I used secondary database files, albeit each one of those was 64bit
IO (approx 70 gigs each).

Are you using the traditional secondary database files mode to create
such a large database or 64bit I/O (single database file) mode?
Were are you geting the data to insert, external files?
I've seen this error reported once before with a large database on
NT/2000 running IB5.6, but we weren't able to track down to source of
the problem.
