I've just uploaded sample application to Files->Bug Testing Applications.
IBO throws Access Violation, when IB_Connection.OnError is assigned. Any sql exception is handled properly then followed by AV. This issue occurs only when IB_Controls or IB_VCL is used in uses clause.
