Yes, it has to be done from the subscribed e-mail address.

Also, I think it should be in the footer, because not everybody views
the header and even thinks to look in there for that information. Or,
even has an e-mail client that has a menu option to unsubscribe from
mailing lists.

Having said that, I've seen on some lists, people requesting to
unsubscribe, when the information is in the footer of every message.
But, probably those people have their e-mail clients stripping out the
footer, or just don't take the time to read.
