Here's a mock up:

---------------------------------------------------------------------------
Client's Name Invoice No Client's Name Invoice No
Client's Address Invoice Date Client's Address Invoice Date

------------------------------------
Lab Order Information |Item No | Item Description | Price|
| | | |
Prescription | | | |
| | | |
Pair of Glasses Information | | | |
------------------------------------
Total
Contact Lens Information
Comments and Other Information:

Doctor's Office Address
---------------------------------------------------------------------------

And, remember this is via an end-user report designer(QR Design),
since none of our reports are hard-coded. The report looks like 2
columns, and all is on one page in landscape orientation. As you can see
the right side looks like a list report and the left looks like a form
report. But, all needs to be part of the same report and a composite
report is not an option, since the original report for the application
looks like the right side of the page. And, in the code I only call
one report to print. Also, the invoice must be easily modifiable by
the end-user.

And, I too have been using QuickReports for 5 years.
