Yes, there is the Survey sample application in IBO that shows how to do
this.
It is really quite a bit of work so I'd appreciate your consideration of a
licensed purchase. I was hired to do this on a contract and put many hours
into it and then the whole project was scrapped and we didn't get paid for
the work. I won't tell you who the company was either.
