Presuming you have double checked all the obvious properites...

Have you tried deleting and re-adding the lookup and/or its dataset?
Only so you can be sure nothing was changed accidently that you are
missing.

You could try posting the relevant section of the DFM here (the
lookupcombo and the lookup dataset) and we can see if we can see
anything.

Of couse the difficulty with this sort of problem is that it is likely
it will be difficult to reproduce in a demo app.
