RDB$INDEX_9 is a unique index on RDB$FUNCTIONS.RDB$FUNCTION_NAME. It seems you are trying to declare a UDF that already exists in RDB$FUNCTIONS.

It seems you have a missing third-party UDF library module, or the library module is in a directory other than /UDF/ which in not configured.
