I'm not 100% sure why, but I noticed odd behavior in some versions of PHP 5, and
when I switched those sites to PHP 4 the problems went away. It could be
coincidence, but I didn't have time to check as that customer is far away and
doesn't have any reliable Internet connectivity.

I have seen situations similar to which you describe in some installations.
However, the load in my apps. is much lower, so it happens rarely. On one of the
sites where it did happen more often I switched to Classic server and I don't
have any problems since. Since you are using PHP as CGI this might be a viable
option.
