Most of the time I have seen this it has been due to user error;
the easy misunderstanding of MS security definitions. Tick the
wrong box and you alter everything in the subdirectories, or
change a parent have have all "inherited" follow automatically.
I should probably thank MS for the money I've made from their
security dialog. :-)
