Dont know if this is going to help but I have seen this problem when using TIBDatabaseInfo through TIBDatabase and also using IBO for data access.

This used to happen on an irregular basis. Could not get it to replicate in any consistent manner.

But we eleminated the TIBDatabaseInfo call and the problem went away.
