Thanks for all the help for this question.

I added a "And Bt.ClientBillToId > 0" to "JOIN
CLIENTBILLTO BT ON SO.CLIENTBILLTOID =
BT.CLIENTBILLTOID". That eliminated the natural index
from the plan and speeded things up enormassly.
