Now we know a bit more to narrow the bug, but still can't see the exact
steps that lead to failure. We'll have to wait for someone "fortunate" to
provide a repeatable failure.

How many connections to the db were in effect? If you don't remember that,
at least do you remember if there was ONE or MANY connections to the same
db?
