It seems that EN_EN and EN_UK are case insensitive by design, but for the
rest, you either sort by upper() and use a couple of explicit collation
instructions (LangRef Chapter 8, Character Sets and Collation Orders,
section Additional character sets and collations) or fiddle with
www.brookstonesystems.com's case insensitive collations.
