Reference, on page 62 shows the "typical TPB declaration" that is:
isc_tpb_write, isc_tpb_read_committed, isc_tpb_no_rec_version,
isc_tpb_wait. It was copied from a reference into the driver. But
since it caused some problems, I've fixed it.
