Quick try resulted in the following errors.....

Error message:
ISC ERROR CODE:335544721

ISC ERROR MESSAGE:
Unable to complete network request to host "interbase2000.org".
Failed to locate host machine.
The specified name was not found in the hosts file or Domain Name Services.

SQL Message:
SQL ERROR CODE:-902

SQL ERROR MESSAGE:
Unsuccessful execution caused by a system error that precludes
successful execution of subsequent statements

I'm behind a very restrictive firewall so this is probably logical..... :-(
Can't map any ports, can't edit my host file and can't ping any ip's
outside the company's intranet....

Once you got the HTML version of the survey up I'll try again....... :-(
If I have any energy left when I get home I'll try it again.
