I just posted a new version of the IBDI Survey app with the suggestions
implemented to the web-site.

I also included the source so that others can take a peek in there and see
how it is all put together. It is really very, very simple.

I use a connection, transaction and a DSQL component in order to plug
information into two stored procedures. The rest of the work is done on the
server in the stored procs.

It is possible for a person to go through the survey multiple times and
replace their former entries if they have a change of mind. Trick is though,
you need to rememeber your password or you will get an error. You also need
to retype the address stuff or it will get lost.
