AH ... first define the problem ;)
Recently I've simply been using the output from Flamerobin, and I've
started filling in the description fields from there, but I run Eclipse
as my development framework and this has a number of options as well.

I've used the free version of Azzurri Clay Database Modeling which
produces some nice graphical output, but we need to convince them that
it should be available outside the Japan :(

What I've been doing recently is simply taking the DDL scripts and
producing 'wiki' pages directly in the website which is quick and easy
to do and allows cross links to be included quickly.

We have been looking at automating some of this for the rest of the
bitweaver framework, but it is still a little 'manual' although the
in-line report generator does work nicely.
