Anything in firebird.log?

Possibly http://tracker.firebirdsql.org/browse/CORE-3285 if you are
using SuperServer and/or http://tracker.firebirdsql.org/browse/CORE-3119
if you are using events.

I would upgrade to 2.1.4 and see if the problem goes away.
