Still trying to solve this one.
On the occasions that Firebird fails to stop, I am getting these messages in the firebird.log:

Shutting down the Firebird service with 1 active connection(s) to 1 database(s)

The database C:\PROGRAM FILES\FIREBIRD\FIREBIRD_2_1\QWLICNS_20100401155206.FDB was being accessed when the server was shutdown
The services that use the databases are all shutdown before Firebird is shutdown, so I assume the Firebird itself is still doing something to the database which stops it shutting down.
I do check the status of all the services before stopping Firebird and they all come back from the service control manager as being stopped, so I don't see why Firebird is failing to stop.
