We have been able to replicate the problem in Firebird 2.1.0 and 2.1.1 superserver in both Linux and Windows environment in different machines. Our development platform is C++ and we are using QIBase driver.

Interesting. Then it does sound like your environment mirrors that
described in the IB bug and you experience the same issue, so it is
quite possible you have stumbled across a bug.
Can you describe your environment a bit more
OS?
Firebird version?
Superserver/ Classic server?
Language/Component set you used to create your stress test?
Have you replicated it on more than one machine?
