well according to the manual which I have cared to pickup to answer this.
Limbo Trans are usually caused by a failure of a two phase commit or a
system failure. If you don't do 2 phase stuff then hardware is likely the
culprit.
There's a whole section on limbo trasnactions in the op guide - I won't
write it all out here. It also describes how gfix can help you
