My 2 cents:
I've been using IB/FB since IB 5.x and FB 1.0x (working on getting to 1.5).
Currently on FB 1.0x using Delphi 6/IBO all on Windows servers and
peer-to-peer networks. I will definitely admit the application was not
written in the cleanest way, it has evolved over several years. I'm working
on code clean-up, especially transaction clean-up as we speak.
I have a fairly low volume of sales but high dollar amount and high
transactions at each sight. It's a full order processing, inventory,
general ledger and financial reporting system. The only time in 6+ years
I've had a corrupted database was due to a bad hard drive. And thanks to
Helen's book, I was able to recover pretty painlessly (just a little
downtime for the users) with no data loss.
I do have automated nightly routines to backup and sweep the database.
I am an independent developer so this kind of reliability is crucial and all
my clients have no IT personnel on-site.

Actually Martijn, those conditions are not far from being the norm for
small to medium business that we deal with on a day to day basis.
Oh the joys of dealing with clients :-) or should that be :-(
