Solution: backup and restore. Simple like that.

No, just got it less bloody. And it's not a bug but something that
works as designed. Just backup and restore with some frequency
and it will never arise again (a backup/restore cycle also do some
cleaning on db -- removes old versions, rebalances indexes, etc --
so it'd a Very Good Thing to do anyway).
