If you are using a release version of Firebird then it indicates some form
of database corruption. A procedure for diagnosing and possibly repairing
corruption is here:

If it is InterBase 6 or an unofficial version of Firebird, then all bets
are off. Bugs in the software have been known to throw this exception on a
good database.

Until you've established what the problem is, and solved it, don't try to
use the database. If necessary, rename it to prevent users from logging
in. Make a file copy of the database for working through the diagnosis and
repair procoedure.
