It looks like a corrupt DB to me.
I don't know exactly what circumstance, but I guess either the version of
the record your transaction would give has been "removed", which should not
happen, or garbage collection is expecting there to be a back version, but
there isn't one there.
I would get everyone off the DB, shut it down and do an os "oh my god" copy,
then go through the standard DB corruption fix routines (gfix, backup and
restore whatever).
