If you download the latest PHP 5.2.x source from php.net and install it
without any manual changes or patching, php.ini does have extension section.

So, it really depends what is one's definition of "normal" :)

You're right, many (most?) distributions choose to change the defaults
for Apache/PHP in order to follow their own filesystem hierarchy...
