Disclaimer: Along with the following links, a Google search for SQLSTATE will reveal quite a few discussions that indicate that there is more than a little bit of inconsistency in SQLSTATE adoption particularly in the case of Oracle.... :-)
Here is IBM's web page for DB2.
http://publib.boulder.ibm.com/infocenter/iseries/v5r3/index.jsp?topic=/rzala/rzalaccl.htm
I think this page is correct for Oracle, even though it's nestled inside something else:
http://www.lc.leidenuniv.nl/awcourse/oracle/appdev.920/a58231/appd.htm
And here is a table of error codes from MySQL:
http://dev.mysql.com/doc/refman/5.0/en/error-messages-server.html
